Analysis of changes across regions
The Recap project executed an analysis of changes across regions including. patient pathways changes, staff experience changes (technology, clinical pathway), changes in prevention metrics across regions and conditions. In this document the implications of telehealth for the care system were discussed with the focus on telehealth related to chronic heart failure and secondary prevention. Here, secondary prevention refers to interventions that happen after an illness or serious risk factors have been diagnosed and the goal is to halt or slow the progress of a disease in its earliest stages. First, telehealth was elaborated upon and defined. Next, the challenges and implications of implementing telehealth were discussed followed by a framework which provides guidance on how to implement telehealth successfully. Best-practices being inspired by the RECAP project were then be described so as to present how this framework can be used in practice. Finally, conclusions and recommendations for policy makers, health professionals, developers and other actors in healthcare were provided.
